Before you start
Have this information ready before you start the Form F86E. You will need to tell us:

- your representative’s contact details (if you have one)
- details of
- the person your dispute is with (the Respondent)
- the regulated labour hire arrangement order the dispute relates to
- what the dispute is about
- steps you have taken to resolve the dispute at the workplace
- what you are seeking to resolve the dispute
- if applicable, any exceptional circumstances that justify the Commission arbitrating the dispute (rather than trying other ways of resolving it first)

What happens next
