A member of a union’s committee of management can apply for a Work Health and Safety (WHS) permit on behalf of the proposed permit holder by completing a Form F42A application.

Who can use this form
The committee of management member and the proposed permit holder each need to complete parts of the F42A application form.
Before you start
To fill out the F42A form, the committee of management member needs the following information:
- the full legal name of the proposed permit holder
- the job title of the proposed permit holder
- the address of the union or branch where we should send the permit if it is issued
- details of the Fair Work entry permit that the proposed permit holder has, or the union has applied for
- details of any previous WHS permits issued to the proposed permit holder.
The proposed permit holder also needs to complete the statutory declaration included within the F42A application form and sign it before an authorised witness.
