Who can use this form
Use Form F99 if you are lodging this declaration with Form F98 – Notice to the Fair Work Commission that a collective agreement has been terminated.
This declaration must be completed together by:
- the regulated business, and
- the registered organisation (like a union)
covered by the collective agreement that has been terminated.
Before you start
Have this information ready before you start.
You will need tell us:
- basic details of the collective agreement that has been terminated, such as its name and the class of regulated workers it covers
- details of the entities covered by the collective agreement that has been terminated
- details of the persons making the declaration on behalf of entities covered by the collective agreement that has been terminated
- the date of effect of the termination of the collective agreement.
