A union official or an employee of a union needs a permit to enter a premises. To renew a current entry permit use Form F42D.

Who can use this form
An entry permit renewal application is made by the union on behalf of the permit holder.
Only use Form F42D if you are:
- a member of the committee of management of a union AND
- applying to renew an entry permit for an official of that union.
You can only use this form if the permit holder meets ALL of the following conditions. They must:
- hold a current entry permit that has not yet expired with at least 4 weeks remaining before it expires to allow time for us to assess the F42D application, and
- continue to be an employee or officer of the same union, and
- have not changed their name and/or signature, and
- look substantially the same as their photo on the current entry permit, and
- have previously provided evidence that they have completed approved entry permit training within the last 6 years.
If any of the above conditions are not met, you must use Form F42 – Application for an entry permit.
Only use Form F42D if you are applying to renew a current entry permit with at least 4 weeks remaining before the current permit expires.
If you are applying for a new entry permit, or the previous permit has expired, you should use Form F42 – Application for an entry permit.
Before you start
Form F42D is a shortened version of Form F42. It does not require lodgement of a photo or evidence of completed entry permit training, because these will be available from when the current permit was issued.
However, the declarations must disclose any changed circumstances relevant to the employee or officer being a fit and proper person (that is, anything that has happened since the previous relevant declarations were made).
To decide whether there are changed circumstances to disclose in the Form F42D application, the committee of management member making the application must make current enquiries, such as directly communicate with the proposed permit holder AND conduct searches on legal databases, Federal Court and Fair Work Commission about the proposed permit holder and assess the result of those searches.
Have this information ready before you begin the application. You will need to give us:
- the permit holder's legal name and job title (or their office if they are an elected officer)
- the address we should send the permit to
- the name of the committee of management member making the application
- your name as the contact for the application (if different from the committee of management member)
- a declaration stating whether that the proposed permit holder is still a fit and proper person to hold a permit. The committee of management member must make current enquiries for this Form F42D application and not rely on enquiries made at the time of the previous application (3 years ago)
- and check that the permit holder has completed appropriate right of entry training within the last 6 years.

Warning about providing false or misleading statement or information
A person who in a permit application:
- knowingly or recklessly makes a false or misleading statement or leaves out information (omits matters) which makes the statement misleading, or
- knowingly gives false or misleading information or omits a matter to make the information misleading,
may be found guilty of a criminal offence punishable by imprisonment. See sections 136.1 and 137.1 of Part 7.4 of Chapter 7 of the Criminal Code Act 1995.
