Who can use this form
Use this form is if you are:
- a registered organisation (like a union), or
- a regulated business
covered by the collective agreement that has been terminated.
Before you start
Have this information ready before you start.
You will need to give us:
- contact details for the regulated business and the registered organisation covered by the collective agreement
- basic details of the collective agreement that has been terminated, such as its name and the class of regulated workers it covers.
When you submit this form, you will also need to provide Form F99 – Declaration that a collective agreement has been terminated.

Email your completed form with Form F99 declaration to lodge@fwc.gov.au.
You can also send or deliver your application to the Commission office near you.
What happens next
As soon as possible after you lodge this form with us, you must send (serve) a copy of this form and any accompanying documents on the other entity that is covered by the collective agreement.
We will register the termination by publishing a notice on our website.
